Key Accountabilities

1. Review prescriptions to ensure they are appropriate and safe for individual patients.
2. Manage the ordering, quality testing, storage, and security of drugs and medications.
3. Supervise the preparation and dispensing of medications according to prescriptions and provide guidance on their proper use, safety, and potential effects.
4. Advise patients and healthcare professionals on medication dosage and the most appropriate form of administration, including tablets, injections, ointments, or inhalers.
5. Participate in ward rounds and obtain patients’ medication histories.
6. Liaise with healthcare professionals regarding medication-related issues and potential adverse effects.
7. Provide patients and their relatives with appropriate medication counselling and guidance on the safe use of medications.
8. Ensure medications are stored appropriately and securely in accordance with SFDA requirements.
9. Provide information and support related to medication expenditure when required.
10. Collaborate with physicians to identify and resolve potential medication incompatibilities or drug-related issues.
11. Utilize available information systems and records to track and understand each patient’s medication history.

Minimum Qualifications

Education & Licensure:

1. Bachelor’s degree in pharmacy or Doctor of Pharmacy (PharmD).
2. Valid SCFHS classification and registration.
