Serves as primary source to coordinate and communicate the financial management, interdisciplinary

clinical service delivery and discharge planning for assigned inpatients and outpatients throughout

the rehabilitation process, which includes assuring a smooth transition into the community or

appropriate discharge destination. The Case Manager will provide professional, competent and

culturally sensitive services to the patients and families of the Humanitarian City.
